Applications

  1. Undergraduate application — for students applying for an undergraduate program, excluding nursing
  2. Undergraduate nursing application — for students applying for our undergraduate nursing program; or Nursing Centralized Application Service (NursingCAS) — for undergraduate and graduate nursing applicants who prefer to apply for admission through our national partner
  3. Physician assistant studies application — for students applying for our combined bachelor of science in human biology and master of science in physician assistant studies program
  4. Undergraduate international application — for international students
  5. Residence hall housing application — This application should be used by students wishing to live on campus or join the Mothers Living & Learning program
  6. Graduate application — for students applying to any Master’s, Doctorate, or Post Baccalaureate Paralegal Certificate
  7. Dual Credit — for high school students who wish to earn both high school and college credit for a course taken at a regional high school
  8. Early College Start  — for high school students who wish to register for regular college courses taught on the College of Saint Mary Campus
  9. Occupational Therapy Centralized Application Service (OTCAS) — for occupational therapy applicants who prefer to apply for admission through our national partner

  1. ​Free Application for Federal Student Aid (FAFSA) — for all students. Complete this form to access all federal, state and university financial aid opportunities
  2. Semester Financial Arrangements — Complete this every semester before classes start
     

Other Forms

  1. Marie Curie recommendation — for Marie Curie Scholars Program applicants to share with the teacher they have requested to submit a letter of recommendation
  2. Physician Assistant Studies Reference — for students needing 3 references when applying for our combined bachelor of science in human biology and master of science in physician assistant studies program

  4. Athletic questionnaire — for students who wish to play sports at College of Saint Mary
