Nursing Adjunct Employment Information

In addition to completing the New Employment Paperwork, the following is required for all Nursing Adjuncts

A copy of your most recent CV/resume should be sent to the appropriate Program Director:
  • Megan Ray, EdD, RN for Undergraduate Nursing employment at MRay [at] csm.edu 
  • Instructor Shamra Lundeen, MSN, RN for PNC employment at SLundeen [at] csm.edu
 
A copy of your unofficial transcript and Plan of Study if you are enrolled in a MSN program should be sent to the Program Director:
  • Megan Ray, EdD, RN for Undergraduate Nursing employment at MRay [at] csm.edu 
  • Instructor Shamra Lundeen, MSN, RN for PNC employment at SLundeen [at] csm.edu
 
Please send all Clinical Requirement below to Stephanie Rupp, Nursing Lab Manager & Clinical Liaison at SRupp [at] csm.edu.
  • Copy of current BLS for the Healthcare Provider (approved by the American Heart Association) certificate to include CPR for the adult, child, and infant as well as AED training. 
  • Recent physical (within 1 year), using the following form. Must include a TB screening with negative result.
  • Proof of Immunizations: 
    • Influenza vaccine (during current flu season)
    • Tdap (given within the last 10 years)
    • Hepatitis B (3 dose series or positive titer)
    • Varicella (2 dose series or positive titer)
    • MMR (2 dose series or positive titer)
    • COVID-19 vaccine (as currently recommended by CDC) with full vaccination status (2-weeks past final dose)
 
Registry check: A link will be sent via email from Stephanie Rupp once the employment contract is requested. This will need to be completed as soon as possible. Results will be sent directly to Stephanie Rupp. The cost associated with the background check is $5 and reimbursement is possible if receipts are submitted to Donna Dawson.
